I reach for RF microneedling when a patient wants firmer, smoother skin with a quick recovery, or has a skin tone where I want heat delivered below the surface rather than ablating the top. I choose CO2 when the surface itself needs resurfacing. They are not rivals — I frequently alternate them in a plan, using RF microneedling for tightening and CO2 for targeted resurfacing.
