“Acne scarring responds best to a layered plan, and I choose based on the type of scarring. For rolling and boxcar scars I use medical-grade microneedling with the DP4 on its scar-treatment (ST) setting, or RF microneedling, to remodel collagen at depth, and chemical peels to refine surface texture and tone. For tethered scars I add subcision to release the fibrous bands tethering the skin down. When scarring is deeper or more extensive, CO2 or Erbium resurfacing does the most. I often combine several of these over a series of sessions rather than relying on any one treatment.”

Acne scars are not all the same, and the type dictates the tool — here is how I actually think about it. Rolling and boxcar scars are collagen deficits, so I remodel them from within with medical-grade microneedling using the DP4 on its scar-treatment (ST) setting or RF microneedling, and I refine surface tone and texture with chemical peels. Tethered scars are held down by fibrous bands beneath the skin, and those need subcision to physically release them before resurfacing can help. When scarring is deeper or widespread, ablative Erbium or CO2 resurfacing does the heaviest lifting. Honest scar work is a series rather than a single visit, so I usually combine modalities across sessions for meaningful, layered improvement — while protecting the skin with sunscreen so new pigment does not set into the healing scars.
