Crepey neck skin is thin skin that has lost collagen and elastin, so I treat it on two fronts — rebuilding the skin’s foundation and refining its surface, and here is how I actually think about it. To rebuild, I turn to collagen stimulation: RF microneedling or Density to heat the deeper skin, and hyperdilute Radiesse, which is beautifully suited to the neck. For the right patient I use my ‘gold protocol,’ Radiesse blended with Belotero and a touch of neurotoxin, to smooth and soften. For surface crepiness in lighter skin that can take downtime, Erbium or CO2 resurfacing does the most. Most of these run two to three sessions, though Density often needs only one to two, and I frequently layer a couple together because the neck responds best to a combined plan. As always, daily sun protection and medical-grade skincare protect the result, because the neck ages quickly when it is neglected.
