GLP-1 medications are prescription drugs, and medical weight loss is a supervised medical treatment — not a cosmetic quick-fix. Before starting, Deborah Lenchus, MSN, APRN reviews your full medical history, current medications and goals to confirm the treatment is appropriate for you.
Boxed warning — thyroid C-cell tumors. In rodent studies, semaglutide and tirzepatide caused thyroid C-cell tumors. It is not known whether they cause these tumors, including medullary thyroid carcinoma (MTC), in people. These medications should not be used if you or a family member have had MTC or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Who should not use GLP-1 therapy. It is avoided in pregnancy and breastfeeding, with a personal or family history of MTC or MEN 2, and with a prior serious allergic reaction to the medication. It is used with added caution in people with a history of pancreatitis, gallbladder disease, certain digestive conditions, or type 1 diabetes. Because of semaglutide’s long half-life, it should be stopped at least 2 months before a planned pregnancy.
Possible side effects. The most common are gastrointestinal — nausea, vomiting, diarrhea, constipation and abdominal discomfort — which often ease as the dose is increased slowly. Less common but serious risks can include pancreatitis, gallbladder problems (including gallstones), low blood sugar (especially when combined with insulin or sulfonylureas), kidney issues related to dehydration, and changes in diabetic eye disease. Tell your provider promptly about severe or persistent abdominal pain or other concerning symptoms.
Medications and formulations. Semaglutide is FDA-approved for chronic weight management under the brand name Wegovy (and, for type 2 diabetes, as Ozempic, which is sometimes used off-label). Some clinics also use compounded semaglutide, which is not FDA-approved and has not been evaluated by the FDA for safety or effectiveness.
